At White Oak we are able to provide you with practical care that enhances your mobility and independence at home. With access to our experienced Occupational Therapists, Physiotherapists and Support Workers, you can help reduce your need for hospital visits or medical intervention.

White Oak’s allied health and therapy services have been designed to keep you confident, mobile and independent in your own home as well as out in your community, doing the things that are important to you.

Occupational Therapy

Whether you’re recovering from an illness or injury, or you need some extra support to keep you safe and mobile, our team of experienced Occupational Therapists can make all the difference.

Our skilled team of Occupational Therapists can visit you at your home to provide assessments as well as practical advice, so you can continue doing your daily tasks your way. Services may include fall prevention advice, home safety checks to reduce risks and hazards, as well as solutions to simplify daily tasks so your safety and independence is maintained. Support services include:

  • Home modifications such as grab rails, ramps and box steps as well as bathrooms modifications such as level access shower
  • Home Safety Assessment to provide home modification recommendations
  • Falls prevention advice and safety in the home (including personal alarms)
  • Pressure management and care
  • Seating assessment
  • Equipment prescription and assessment
    • Mobility – wheelchair, walking frame, scooter
    • Personal care
    • Kitchen.

The extensive support and advice offered by our Occupational Therapists can make all the difference by helping you prevent falls and achieve a safer, more accessible home.

Physiotherapy

White Oak’s own team and Physiotherapists are experienced and passionate about delivering quality therapy to our clients, in the comfort of their own home. Our allied health team work closely with our nursing team and home care coordinators, so as to ensure your specific needs are understood and the very best care is provided.

Support services include:

  • Comprehensive initial assessment conducted to guide direction of treatment and discussion of goals
  • Physical assessment of body structures and function (posture, mobility, balance, strength and endurance)
  • Assessment of home environment and falls risk
  • Exercise therapy to improve balance, strength, exercise tolerance and mobility
  • Falls prevention education
  • Pain management education and intervention including soft tissue mobilisation
  • Rehabilitation after surgery or hospitalisation
  • Walking aid and device prescription

Arranging Allied Health Care Services

You can arrange allied health services from us at any time as a service that you purchase privately. We charge an hourly rate, book in blocks of thirty minutes, and can arrange this as you need it or as part of a structured program you would like us to plan for you. You do not need to commit to a contract or a minimum number of visits, and you can adjust your services at any time. You can discuss your options with us at any time.

If you have a home care package, you are able to get allied health services from us at any time by using the funds available from your package. If you are an existing client we can arrange this through your current support plan. If your package is managed by yourself or by another provider, simply contact us to arrange allied health services for you and we will invoice either you or your provider as you wish.

We can deliver all the allied health services that you are eligible for based on your assessment in the Community Nursing program that we are approved to provide by the Department of Veterans’ Affairs.

FAQs

What is home care?

Home care is a range of care and services that help you, or the person you care for, to continue living at home for as long as possible.

Services range from weekly support around your home to a daily visit to provide personal care, meal preparation, cleaning medication assistance or regular nurse visits.

You will have a dedicated Home Care Coordinator to support you on your journey.

How much does home care cost?

Home Care is a Government-subsidised service. You may be required to contribute to your funding, depending on your individual circumstances and the services you chose. Your Home Care Coordinator will work with you to create your budget. All unspent funds roll over each month.

How do I know which home care services I’m eligible for?

The Aged Care assessment team (ACAT) will check your eligibility to access Government-funded aged care services.

To be eligible they will consider your needs, medical conditions and diagnosis, supports available to you, etc.

Your needs may be considered temporary or long term, which will determine if you are eligible for the Commonwealth Home Support Programme (CHSP) or Packaged Care.

We can help you to identify which services and supports are available for you.

What is an ACAT Assessment?

An aged care assessment, or ACAT assessment, is required for people who want to access Government-funded aged care services. The assessments are organised by an Aged Care Assessment Team (ACAT). They are professionals with medical backgrounds, such as doctors, nurses, occupational therapists and social workers.

An ACAT assessment is an important first step towards accessing aged care services. The assessment is conducted by trained professionals who want to help you access the support you need to live your best and most fulfilled life.

The assessment is conducted to determine your needs so you will always have the opportunity to voice your concerns, ask questions, and make your own decisions.

Not all aged care services will require an ACAT assessment. You only need to be assessed if you are using services that are subsidised by the government.

What should I expect in my ACAT assessment?

Times can vary, but assessments usually take between 45 and 75 minutes. An ACAT team member will visit you to gather all the information, asking a series of questions about your day-to-day life and health. They may ask you how you’re managing with routine activities and if you require help with them, what support you have from family or friends, what health conditions you have or medications you take, how you feel your health is and whether or not you are having memory problems.

They’ll also make sure you understand the process of receiving aged care and discuss your options going forward. After your interview, they’ll talk to the rest of their team and make a decision about the level of care you are eligible for.
