Eligibility

You may be eligible to receive home care services through a Commonwealth funded program. As an accredited provider of Home Care Packages, Veterans’ Home Care, Community Nursing and the Commonwealth Home Support Program, White Oak can deliver services to you at a subsidised rate.

To have your eligibility determined, you will need to arrange an assessment from the Government’s Age Care Assessment Team that can be arranged by calling My Aged Care on 1800 200 422.

If you are approved for a home care package, or would like to be, then your first step to getting services with White Oak is to contact us.

What level of package is right for you?

A level 1 or 2 package gives you enough funds to provide a bit of support at home. A level 3 or 4 package offers a higher amount of funding so you can manage at home as long as possible.

The level of package offered to you by the Government will be based on your assessed needs.

Getting services outside of a home care package

If you are unable to access services within a homecare package, dont let it stop you from receiving quality care.

While you wait

There may be a waiting period from when you are approved for a home care package and when you are assigned one.If you feel that you need some services right away, then we can look at how else we can get you started with some help around your home to ensure you can manage while you wait. We might be able to find a low-cost seniors program if it is respite that is needed for your carer, or we can look at getting someone into assist you with your personal care and housework if needed.

If you are not eligible

If you are not eligible for a home care package, we can also look at setting up a pay as you go arrangement or see if there are any other programs available that could assist.

Topping up your home care package

We can also put this in place to top up your services if your package does not cover all the services that you would like to have each week.
